Immunization Requirements
The state of Oregon requires children to receive certain vaccinations to attend public and private schools. Here is the updated list of the vaccines required by age group.
- If your child will be new to ISP this fall, please send a copy of their updated immunization record to the school prior to the first day of school.
- Families with returning students who have received vaccinations since last year should also send an updated record to the school prior to the first day of school.
Looking for the Oregon Certificate of Immunization Status (CIS) form?
Please note that immunizations can come due at any time during the year based on your previous immunization schedule. Exclusion day is in February and any child that has not already had all required immunizations will be excluded from school until they are up to date, so please take note of your child’s immunization status and schedule accordingly.
If you are claiming a nonmedical exemption to vaccines for your child, please note that Oregon Immunization Law has recently changed. You will need to review the updates to the law that affect how this exemption can be claimed. More information can be found here.

Vision & Dental Requirements – New Students
In addition to immunization records requirements, Oregon Law now requires that a child who is seven years of age or younger to have vision and dental screenings before entering school for the first time. Please note that religious exemption(s) may be accepted for vision and/or dental screenings only.
Click here to fill out the Vision and Dental Screening Certification for New Students, sign both, and return it to the school. Please bring your completed form to Stearns Hall front desk, or email a copy of your signed form to nurysh@intlschool.org
