Immunization requirements

Dear Parents,

Oregon law requires that all children attending school, preschool or daycare be up-to-date on their immunization (shots), unless they have medical or religious exemption on file. If you have already submitted the immunization form to TIS (which you did if your child was here last year) and if your child has received an additional vaccination, please just come in to update and re-sign the form before September 18, 2009 . The forms are with the administrative assistant on the middle floor of the main building.

If you have not submitted a form - click here for the Certificate of Immunization Status (CIS) form. It is the document for immunization that the state recognizes. Thus, it cannot be substituted with doctors’ or medical records.

If your child/children are exempt from any of the vaccinations, please follow the directions on page 2 of the CIS form. If your child/children have received all of the doses of all of the vaccinations, please fill out and sign the form; then bring it to Registration (new families), send it in with your registration papers (returning families)  or return it to the Front Office before September 18, 2009.
